Depending upon the time of reconstruction, they are divided in two types:

  1. Immediate reconstruction- Here breast is reconstructed immediately after the mastectomy surgery. This gives psychological benefit to the patient of getting them reconstructed under the same anaesthesia.
  2. Delayed reconstruction- Here the breasts are reconstructed after a period of 6 months to one year depending upon the adjuvant therapy planned for the patient.

Procedures:

This is usually a two-or three-step process.

In the initial procedure, your surgeon inserts a tissue expander beneath the skin and chest muscle, forming a skin-muscle envelope. The tissue expander is a modified saline implant with a valve, allowing more saline to be added after the first surgery. Serial injections of saline through the skin into the valve slowly fill the implant and will subsequently expand your breast mound. During office visits over two to six months, the skin-muscle envelope is slowly stretched until it reaches the size you want for the final implant.

In the next stage, you will undergo outpatient surgery during which the expander is removed and replaced with a softer breast implant (saline or silicone).

It is rare for a woman to have an implant (saline or silicone) inserted directly without first having tissue expansion. In this situation, the size of the skin-muscle envelope at the time of mastectomy is large enough to cover the desired final implant.

Reconstruction using patient’s own body tissue is the most natural way. It gives psychological benefit to the patient as no artificial material is used. The most common tissue used is abdominal tissue (DIEP flap). The extra fat and skin present over the abdomen are excised along with their blood supply. The abdominal skin is then transferred, and its blood vessels are sutured with those of the chest. Patient gets the added advantage of tummy tuck being done along with the other tissue which can be used are from the gluteal region or from the inner thigh.

Fat grafting is another option for breast reconstruction. Here the fat is removed from abdomen or thigh and transferred with the help of cannula to the breast. Autologous fat grafting or fat transfer is another option for treating radiation-damaged tissues or small areas of contour irregularities.

The nipple is usually recreated by lifting a flap of skin from the reconstructed breast itself and folding it in such a way as to create a small piece of tissue with projection. There are several methods to achieve this and most can be done in an office setting under local anaesthesia.
